TREATMENT OF CHRONIC LYMPHEDEMA OF THE PENIS AND SCROTUM

Abstract

|
Lymphedema is a collection of lymph in interstitial tissue resulting from a functional overload on the lymphatic system. The overload is the effect of slowed intralymphatic flow due to maldevelopment of lymphatics, lymph node blockade, or venous stasis.
Elephantiasis of the penis an-d scrotum may be primary and secon-dary. These causes due to primary causes-aplasia or hypoplasia of the lymphatisc-are considered relatively uncommon. Secondary etiological factors are thought to be responsible for the majority of cases.
Chronic lymphedema of the penis and scrotum is a serious condition that causes the patient physical as well as psychological impairment. Patient with this condition lose sexual function. Theymay also lose family and friends, and become social outcasts. This is especially true in young men. The ultimate goals of reconstruction include the restoration of cosmetic nature and the preservation of sexual function. The basic techniques include: 1) lymphangioplasty; 2) lymphatic bridge procedures with pedicles; 3) transposition of an omental flap; 4) lymphaticovenous shunt.
In this paper, one case of satisfactory reconstruction of the scrotal and penile lymphedema, treated by excision of lymphedematous tissue and local skin flap, was reported with some reviews of the relating references.
